Thank you for scheduling an initial consultation with Ohio Naturopathic Wellness Center. Here is all of the information you need prior to your appointment.
Prior to Your Appointment
1- Complete your health history form. You can click the button below to go to our health history form page where you can either download and print your health history form or you can complete Dr. Ted’s history form online. If you have any trouble with the forms, please call or text us at 330-729-1350 and we can mail out a history form for you to complete. If you can’t complete your history form prior to your visit, please come to your appointment 30 minutes prior to your scheduled time.
2- Pay your $50 new patient appointment deposit. We require first time patients place a $50 deposit on their appointment to secure their visit time as a good faith gesture that you will keep your appointment. Since your initial consultation is scheduled for 90 minutes, if you cancel last minute we may not have time to offer this appointment to another patient. Failure to pay your deposit may result in the loss of your appointment. To make the deposit online click the button below. You can also call the office to make deposit over the phone, mail in a check, or drop off deposit to the office.
3- Gather any recent blood lab reports or other test reports to bring in for your appointment.
4- We have a 24 hour cancellation policy and ask that if you must reschedule or cancel your appointment, do so at least 24 hours in advance to avoid a $50 cancellation fee.
What to Bring to Your Appointment
- Please bring any medications and supplements you are taking with you to your first appointment, along with blood work results or any other lab results.
- Payment for initial consultation and any supplements that may be recommended.
- An open mind, ready to provide your doctor with all of the information they need to help you, along with the willingness to make the changes needed to be successful in this new journey.
